All safety management systems should include a process of internal and external audits to assess their ongoing effectiveness in reality. There is a real danger that an inappropriate approach to audit can have significant unintended consequences on the safe behaviour of your teams.

Peer observations can be more powerful than official audits in many situations, and a simple ‘check properly check’ approach may be all that is required from a management perspective.
