Equally important, is a clear and transparent investigation process, that considers all causation factors and provides feedback to the teams to demonstrate the genuine commitment to learn and improve.

We recognise that human beings make mistakes. An effective investigation process will not automatically ‘blame’ the front line workers, but will endeavour to identify the root causes, which may be a result of violations or may equally lie with the organisational processes or leadership.
ABC analysis and Swiss cheese theory are extremely useful tools to help identify why things went wrong, and an effective behavioural safety programme will attempt to re-design the antecedents and re-align the consequences to stop the wrong behaviours and/or encourage the right behaviours.
